The Community Health Worker Collaborative Network is changing the face of community health. The Network connects culturally, linguistically, and community specific community-based organizations to funding for CHW services with the ultimate goal of increasing workforce sustainability and enhancing continuity of services that community members receive.

As frontline public health workers, CHWs are uniquely skilled at helping community members understand how our health and social systems work, ensuring people receive wrap-around support they need—when they need it. Unfortunately, many CHWs struggle to make a livable wage or find job security despite their vital work.

Through the CHW Collaborative Network we have developed a strategy to increase the sustainability of the CHW workforce.
